 How to Generate some Performance parameters.

I need to generate a report that will comprise the following properties:
timestamp, No of CPU, CPU usage in 5 hours, Run QueueLength, Total IO,
Memory Size, Memory %usage and Paging Rate. Limited time is given to achieve
this task. Thanks.
