I was wondering if anyone has a list of questions (or a link to them) that they would ask a DBA who is leaving to make sure you can smoothly transition their work.

Any list questions would be helpful, big or small. I have been making my own list of questions, but I don’t want to wait till after they are gone to realize I forgot to ask something.

Any examples of transitions that went wrong would be helpful also.

How big is your organization? If it's of any decent size, then everything the DBA does should be well documented in a set of SOPs that can be handed to then new DBA.

That documentation does not exist in this case.

In any case, there is always information that exists only in someone’s head, so I am looking for questions to ask that take advantage of the brief window I have to get these questions answered.

It really does depend on the organization. I suppose the most general all purpose question would be, who fills in for you when you go on holidays, and what things do you normally have to inform them of before you leave?

- If you had time, what would you have changed before you left?
- Any nasty triggers I should know about?
- Are backups in order and where do I find them?
- Give me a list of 5 things that are likely to blow up in the forseeable future, in order
- Who are the important names in your telephone list, and what are their numbers?
- Which systems take up the most of your time and why?
- Who in the company contacts you directly to do stuff out of the ordinary and why?
