Is SQL Profiler the tool used to generate graphs like

If so, how is Profiler configured to generate this particular graph of open connections?

The article says he's using Performance Monitor to generate the graphs. I don't think Profiler graphs anything by itself, but you might be able to take its data and graph it in something like Excel.

Would be nice if the graphs were integrated.

I don't see any way to monitor connections in SQL PM. It must be disguised under another name.

The graph is from Performance Monitor and not from SQL Profiler.

Performance Monitor is not a SQL Server tool. It comes with Windows. You will find it in the Administrative Tools program group.
